I think that even if it's full, if two people try to switch at the same time, it will work. If both people click to try and switch teams/positions at the same time, it will work. It's no hack, no program as far as I know.

No I am 100% positive that this was a program.

My brother's clan leader has it, it's pretty amazing.
The bot can host games, then switch players around.
It can randomize teams, too.
